The mission of Community Health Center is to improve the health of our community through the provision of comprehensive, high-quality, compassionate health care to all, regardless of ability to pay. CHC provides care to more than 23,000 individuals within a "complete medical home" model, where multidisciplinary, integrated health services are provided under one roof to assist patients in overcoming affordability, financial, linguistic, cultural, transportation and other barriers to care. This complete medical home model includes primary care, behavioral health care, dental care, women's health services, optometry, substance use disorder services, on-site affordable prescriptions, x-ray and ultrasound services, case management services, barrier assessments and assistance, Veterans care, and many other specialized programs. In 2019 and again in 2020, Community Health Center was ranked in the top 10% of health centers nationally for clinical quality by the Health Resources and Services Administration (HRSA).
